CommuniCare Health Centers is a Federally Qualified Health Center, offer a sliding fee scale discount on services if you are ineligible for health insurance.

The discount is based on household income and family size.

Dental Hours: 


Monday – Friday, 8:00 am – 5:00 pm. 
Evening hours every other Wednesday until 9:00 pm. 
Evening hours Thursday until 8:30 pm.
